Te Omanga Hospice is a charitable trust providing specialist palliative care to patients and their whānau living with a life‑limiting or terminal illness in the Hutt Valley. We are seeking a specialist Palliative Care Registered Nurse to join our Community Nursing Team. This team delivers high‑quality specialist palliative care to people in their homes.

About the Role
As a Community Specialist Palliative Care Nurse, you will provide comprehensive, holistic nursing care to patients with complex palliative needs, working autonomously while supported by an experienced multidisciplinary team and primary care providers.
This role requires advanced palliative care knowledge, confidence in community‑based practice, and the ability to build strong therapeutic relationships with patients, whānau, and primary care providers. You will contribute to symptom management, care planning, patient and family education, and coordination of services across settings.
The position offers flexibility and professional autonomy within a supportive specialist service and is available at 0.8–0.9 FTE.
Key Responsibilities
- Deliver specialist palliative nursing care to patients in the community
- Undertake comprehensive assessments, care planning, and symptom management
- Support for patients and whānau to remain at home where appropriate
- Collaboration within the multidisciplinary team and with external health providers
- Maintain timely, accurate, and professional clinical documentation
- Participate in service development, quality improvement, and clinical education
Desired Skills and Experience
About You
You will be an experienced specialist Palliative Care Registered Nurse with a strong interest in community‑based care and the confidence to work both independently and collaboratively.
Essential Requirements
- Registered Nurse with NZ Nursing Council registration (or overseas equivalent)
- Current Annual Practising Certificate
- Demonstrated specialist palliative care knowledge
- Experience working in a community nursing setting
- Ability to assess, prioritise, and manage complex patient and whānau needs
- Strong communication and relationship‑building skills
- Commitment to holistic, patient‑ and whānau‑centred care
- Understanding of cultural, spiritual, and grief considerations in palliative care
- Full, clean New Zealand Driver’s Licence
- Computer literacy and confidence with clinical documentation
Desirable
- Postgraduate qualification or working towards postgraduate study in palliative care
- Experience supporting patients with life‑limiting illness across diverse settings
