About The Initiative
Comprehensive Primary Care Team - Practice Pharmacist
ProCare's Comprehensive Primary and Community care Teams (CPCT) model has been successfully operating for almost 2 years. This initiative integrates allied health professionals into general practice teams to improve equity, reduce barriers to care, and support our most at-risk communities. The programme provides free access for patients to a range of services, easing pressure on GPs and enabling holistic, whānau-focused care.
About The Role
The Practice Pharmacist will serve as the subject matter expert within the practice on all matters relating to medicines information and advice. You'll work directly with patients to provide individualised pharmacotherapy guidance and deliver education sessions to patients and whānau, and raise awareness of medicine-related harms, among other responsibilities.
As a CPCT Practice-Based Pharmacist, Your Responsibilities Will Include
- Medicines management, optimisation, and reconciliation
- Clinical evaluations and implementation of best practice
- Medication evaluation, audit, and quality improvement to support equitable access
- Development and oversight of standards, policies, and procedures for safe, effective, and equitable medicine use
- Active participation in an IDT (interdisciplinary team) structure, using systems and processes that enhance team functioning
- Building and strengthening partnerships with other health and wellbeing providers
We're Interested in Pharmacists Who
- Hold a current APC with no restrictions
- Have 2+ years' experience as a registered pharmacist in primary or secondary care
- Have completed or are pursuing postgraduate studies in clinical pharmacy or pharmacist prescribing
- Possess a strong understanding of health inequities in Aotearoa New Zealand and the application of Te Tiriti o Waitangi in primary care
- Demonstrate cultural competency and confidence, with knowledge of the peoples, languages, and cultures of our most at-risk communities
- Have experience working in collaborative, multi-disciplinary health teams (advantageous)
- Are experienced in supporting long-term condition management through initiation and continuation of prescribed treatments (highly regarded)
