About The Job
We need a registered nurse with two years’ clinical experience in Palliative Care Nursing to join our specialist services team on a 6-month fixed term basis as a Triage and Referrals Nurse. This role will be full time (1.0 FTE) Monday to Friday. We will provide professional development opportunities and supportive leadership.
Reporting to the Director of Specialist Services, the objectives of the role include:

You will be a problem solver, calm under pressure with exceptional communication skills and sound nursing experience. With a background a strong nursing background and excellent planning skills you will be confident to hit the ground running.

About The Job
We need a registered nurse with two years’ clinical experience in Palliative Care Nursing to join our specialist services team on a 6-month fixed term basis as a Triage and Referrals Nurse. This role will be full time (1.0 FTE) Monday to Friday. We will provide professional development opportunities and supportive leadership.
Reporting to the Director of Specialist Services, the objectives of the role include:
- Working within an interdisciplinary team (IDT) using advanced clinical expertise in assessment, care planning and service delivery for patients and their family/whānau.
- Attending a daily referral IDT meeting to discuss and triage all new referrals.
- Identifying the needs of new referrals to ensure an appropriate response to the patient’s care needs.
- Making contact with patient and family/whānau to outline our services and supports and arrange a time to engage in the community or in clinic at our Hospice.
- Strong focus on ensuring a smooth transition from hospital back to the community. Working collaboratively with all care teams involved.
- Managing the daily triage phone offering advice, support and education regarding specialist palliative care to patients, family/whānau and other health providers.
- Maintaining strong relationships with primary and secondary health care providers.
- Participating in ongoing professional development and training opportunities
About You

You will have the following skills and experience:
- Registered Nurse with a current New Zealand Annual Practising Certificate
- A minimum of 2 years’ recent clinical experience in Palliative Care Nursing (at the CNS Grade 1 level)
- Exceptional interpersonal / relationship building skills with people from all walks of life.
- A current PDRP at Expert Level (or working towards)
- Excellent communication skills
- A willingness to learn and develop
