About the Role
The role of the BBHB team is to support patients with a diagnosis of respiratory or heart failure, to improve their exercise tolerance, their understanding of and manage their disease process. The Pulmonary and Heart Failure rehabilitation programme allows patients to maintain quality of life, independence and helps to reduce hospitalisations. The HCA role within the team is to assist the registered nurse and physiotherapy staff to achieve these outcomes. The programmes are held in community gyms, patients homes and virtually. The work is varied, rewarding and interesting. If you have not previously worked in the community environment, this a wonderful opportunity to support our community to understand and independently manage their health requirements.
About the Team/Service/ Location
The wider Community Nursing Service teams consist of the District Nursing teams in Napier and Hastings, the Continence, Ostomy, and the Wound and Community Clinical Nurse Specialist teams, as well as Clinical Nurse Coordinators, Health Care Assistants and administration team support. The services are based at Hawkes Bay Fallen Soldiers Memorial Hospital and Napier Health Centre. Referrals are received from the primary and secondary health sectors.

About you
You will need the following key attributes and skills:
- Previous experience working in a health care setting
- Full unendorsed drivers’ licence
- Computer skills
- Exceptional customer service skills
- Reliable and trustworthy
- Previous experience working within a team
