About the Role
As a Clinical Nurse Specialist – Heart Failure, you will take a lead role in the provision of specialist nursing care and coordination for patients living with heart failure. You will work collaboratively with multidisciplinary teams to support optimal disease management, patient education, and improved outcomes across both hospital and community settings.

About the Team/Service/ Location
Our Cardiology Services team provides comprehensive, patient-centred care across the continuum of acute and chronic cardiac conditions. We are a collaborative, passionate team dedicated to excellence in clinical practice and innovation.
As a Clinical Nurse Specialist – Heart Failure, you will bring a high level of clinical expertise, a passion for delivering patient-focused care, and the ability to lead and support both patients and colleagues. You will also demonstrate a commitment to equity and culturally safe practice.
- A current Clinical Nurse Specialist or Registered Nurse with a current Annual Practising Certificate and post-graduate qualifications (or working towards) in cardiac or advanced nursing practice
- Is a current RN Prescriber or a commitment to be working towards becoming an RN Prescriber within primary care or specialty team.
- Minimum of 5 years’ post-registration experience, with recent experience in cardiology or heart failure care
- Demonstrated leadership, advanced clinical decision-making, and excellent communication skills
- Proven ability to work both independently and collaboratively within multidisciplinary teams
- Commitment to continuous quality improvement and patient-centred care
