About the role
The Community Rehabilitation, Enablement and Support Team (CREST) based in Christchurch is a dynamic, fast-paced, community-based team providing short-term rehabilitation and enablement interventions to reduce the number of admissions of older persons to hospital and residential care. Acting as a bridge between referrers, clients, their families, and our CREST team, we create effective enablement or rehabilitation plans. The CREST team also supports patients to leave the hospital and continue their recovery at home.
About You
We are looking for an experienced Allied Health professional or registered nurse, passionate about supporting people to live well at home. The successful candidate seeks a new challenge in service provision to grow their clinical skills, and improve patient outcomes while working as part of a dynamic and high-functioning team.
In this role, you will establish rapid management plans focused on supporting rehabilitation and enablement through self-management, optimizing patient goals, and independence. This role will also involve daily triaging of referrals to the allied health and nursing team, as well as our colleagues in home support provider services, ensuring that each client receives timely and appropriate support.
You will be expected to act as an ambassador for our service, remain calm and confident when dealing with challenging situations, and collaborate across an interdisciplinary team; communicating with various parties to ensure a seamless service delivery.
Experience working with older people is essential; experience in the mental health sector is an advantage.
This is a designated position on the Allied Health and Technical salary scale.
To be considered for this role, you need to hold a current Annual Practicing Certificate in an allied health profession or a current registered nurse practicing certificate with no restrictions.
