About the Role

We have an exciting opportunity to join the Diabetes Specialist Service as a Diabetes Clinical Nurse Specialist. Our team is dedicated to empowering people to manage their diabetes. The 0.6 FTE permanent position offers a role in a clinical leadership position to shape strategic direction and deliver a holistic level of care.

As a senior member of the team, you will focus on and embrace connectivity by:

  • Working with communities to build, maintain, and enable strong foundations for the health and wellbeing of patients to manage their diabetes.
  • Prioritising disease prevention, health promotion, and health & wellbeing.
  • Understanding the needs of patients and communities, enabling them to exercise authority over their health and wellbeing.
  • Growing and continually developing the Diabetes service and its care delivery to a high standard of care.
  • Empowering patients to manage their diabetes and optimise their health.

About the Service

The Diabetes Service provides an outpatient consultation service for patients on referral from general practitioners and other health professionals. It provides ongoing surveillance of diabetes management and complications status for selected patients who are at higher risk of developing diabetes complications. The Service also provides medical and diabetes nurse specialist consultation services to the hospital inpatients.

The service has a multi-disciplinary team consisting of Diabetes Physicians, Diabetes Clinical Nurse Specialists, Dietitians, Psychologists, Podiatrists, Māori Diabetes Nurse Specialist/RN, a Pacific Island Diabetes Nurse Specialist, Clinic Nurse Educator, and a Diabetes Adolescent Nurse.

About You

  • A Registered Nurse with at least five years of diabetes nursing experience or a relevant nursing role.
  • An expert at leading and developing nursing practice to improve health outcomes and the management of diabetes.
  • Excellent interpersonal skills and ability to build effective relationships with a strong patient focus.
  • Ability to work collaboratively in a multidisciplinary team.
  • Relevant postgraduate qualifications, with a view to obtaining a Nurse prescribing qualification is desirable.
  • Registered with the Nursing Council of New Zealand with a current Annual Practising Certificate (APC)
