About the Role

Supported Transfer and Accelerated Rehab Team (START) is an early supported discharge programme that accepts referrals for patients who are medically cleared and can be discharged home earlier than previously possible with an individualised care plan and intensive holistic support.

START provides up to four visits a day, 7 days/week using Health Care Assistants with oversight from Registered Nurses, Physiotherapist, Occupational Therapist and input as required from Medicine / Geriatricians and other specialties e.g. SLT.

START is part of LICHS (Lakes Integrated Community Health Service) which includes our Needs Assessment and Service Coordination services and District/Community Nursing. LICHS sits as part of the Medicine Sercvice.

We are seeking a dedicated and flexible clinical professional to join our team in a 1FTE, 40-hour per week role. This position requires flexibility so you may sometimes be called after hours or on weekends.

Ideal candidates will have a strong work ethic and be super-committed to our early supported discharge service, leading a team of clinical professionals to deliver an outstanding 7 days/week service.

About you

You should:

  • Have a Health qualification with a current New Zealand practicing certificate and with the equivalent of at least five years’ recent relevant experience. Be a competent clinical professional capable of managing a team of varied clinical professionals and working collaboratively with other multidisciplinary teams.
  • You will have a broad range of experience both clinical and managerial.
  • Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth is desirable as is experience in projects / initiatives which give effect to Te Tiriti principles and frameworks, and the application of Mātauranga Māori and Kaupapa Māori approaches, particularly as they apply in healthcare settings.

Salary range

$117,420 to $122,570 pro-rated to hours worked
